must be on one (1) original receipt. If receipt does not specify product names include the original proofs of purchase (UPC's) along with your receipt and official
form. Place in a stamped envelope and mail to: P&G Frames/Lenses Refund, P.O. Box 5534, Clinton, IA 52733-5534.

Qualifying Procter & Gamble purchases must be made by 9/15/06 AND purchase of frames and lenses from Sears Optical must be made by 11/30/06, and completed
request must be postmarked by 12/15/06 and received by 12/21/06. Void where restricted or prohibited by law. Void in Oklahoma, Puerto Rico, La Jolla
and El Cajon, CA and where prohibited.
